Ecumen Hospice seeks a Nurse Practitioner to join its team providing services in Chisago County and the surrounding communities! Ecumen Hospice honors life through care and serves individuals in the Twin Cities – Metro Area. We are excited to add a Nurse Practitioner to our dynamic, growing team in the Chisago County and surrounding area! The Nurse Practitioner will be directly responsible for conducting the hospice face to face recertification as needed. The primary role of the Hospice Nurse Practitioner is to travel to the residence of the hospice patient and perform hospice face to face visits prior to the third and later hospice certification periods by assessing hospice eligibility based on the hospice conditions of participation and the local coverage determinations. The Hospice Nurse Practitioner will provide clinical information to the hospice medical director for interpretation for eligibility and make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
