Overview

Ecumen Hospice seeks a Nurse Practitioner to join its team providing services in Chisago County and the surrounding communities!  Ecumen Hospice honors life through care and serves individuals in the Twin Cities – Metro Area. We are excited to add a Nurse Practitioner to our dynamic, growing team in the Chisago County and surrounding area!  The Nurse Practitioner will be directly responsible for conducting the hospice face to face recertification as needed. Responsibilities

The primary role of the Hospice Nurse Practitioner is to travel to the residence of the hospice patient and perform hospice face to face visits prior to the third and later hospice certification periods by assessing hospice eligibility based on the hospice conditions of participation and the local coverage determinations. The Hospice Nurse Practitioner will provide clinical information to the hospice medical director for interpretation for eligibility and make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. Specific responsibilities include:

  • Perform hospice face to face visits prior to the third and later hospice certification periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.
  • Assess hospice patients by reviewing the clinical information about the patient and when appropriate, provide documentation that it is anticipated that the patient’s life expectancy is 6 months or less if the illness runs its normal course.
  • Comply with Ecumen hospice program’s policies and procedures, and help Ecumen continually evaluate and improve, when prudent, its policies and procedures.
  • Comply with all state and federal laws and regulations related to providing hospice care.
  • Review, respond and participate in any external surveys or inspections.
  • Participate in the hospice program’s quality assessment and performance improvement program on a limited basis.
  • Follow and promote compliance with Ecumen’s established procedures and safeguards for the protection of resident privacy and confidential health information, including but not limited to Protected Heath Information as defined under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).

Qualifications

  1. Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
  2. Current nursing licensure in State of Minnesota and CPR certification.
  3. Accreditation from an approved certifying body for advanced practice nursing.
  4. Must possess a valid driver’s license and driving record which meets Ecumen Driver Policy standards and proof of vehicle insurance.
