    Ecumen Hospice is looking for a full time Registered Nurse (RN) Case Manager to join its growing, dynamic team providing services in Chisago County and surrounding communities!  The Ecumen Hospice team's mission is to provide care that exceeds the expectations of those that find themselves in the later stages of life by providing a level of comfort and quality of life that makes each person feel honored and supported as their life continues to have significance. 


    The RN Case Manager is tasked with the responsibilities of: delivery of patient care, supervision of hospice aides, education of patients and their caregivers, and participation in quality assurance of patient care initiatives. Specific responsibilities include:

    • Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness.
    • Assesses and evaluates patient’s status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs as well as participating in revising the plan of care as necessary
    • Utilizes health assessment data to determine nursing diagnosis.
    • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
    • Initiates appropriate preventive nursing procedures.
    • Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
    • Counsels the patient and family in meeting nursing and related needs.
    • Provides health care instructions to the patient as appropriate per assessment and plan
    • Other duties as assigned




    This position requires a person who is a graduate of an accredited school of professional nursing. One or two years of nursing experience in post acute care setting is required. One to two years of nursing experience in Medicare Hospice is preferred. All Ecumen employees must possess the ability to communicate effectively, including written and verbal skills; work well, both individually and as part of a team, through respect, collaboration and dedication; and successfully interact with a diverse culture base. The successful candidate must be able to prioritize and organize work effectively and efficiently, and demonstrate a positive attitude, high integrity and ethical awareness.
