This position is responsible for providing general clerical and administrative support to staff and clients of the organization.  The main responsibility of the Hospice Health Information Specialist will be the maintenance of the agency’s medical records as well as assisting in inventory and purchasing of office and medical supplies.  This position is also responsible for intake and insurance verification. 


Essential Job Responsibilities Include:

  1. Organizes medical records activities including assembling new charts, accurate filing (by patient/admission in chronological date order), purging and disassembling of charts post discharge
  2. Provides office support to the clinical team such as tracking orders, coordinating referrals and admissions
  3. Greets visitors and telephone callers in a prompt and courteous manner. Able to take accurate messages and discern priority and urgency of the customer’s need
  4. Coordinates all departmental office support responsibilities such as the Interdisciplinary Group meetings
  5. Verifies patient payer sources and schedules all hospice and home care admissions in software system and ensures accuracy of information
  6. Composes, types, copies, and/or disseminates routine and confidential correspondence as requested and approved
  7. Other duties as assigned.


Health Information Specialist

Minimum Required Qualifications:


  • High School Diploma or GED
  • Up to 3 years of related experience in home care/hospice
  • Ability to communicate effectively in both written and verbal formats
  • Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, Resident’s Bill of Rights, HIPAA, OSHA, Universal Precaution, AWAIR, Right to Know, and MSDS
  • Proficient in Microsoft Office


Preferred Qualifications:


  • Prior experience working with seniors
  • Experience with electronic medical records systems
