We are looking for an LPN to work PT-NOC Shift, (30 hours per pay period - 1 shift per week and everyother weekend).

The Licensed Practical Nurse (LPN) performs a variety of professional nursing duties in the care of home care clients, complies with state and federal rules and regulations, and follows established policies and procedures of the agency.  Implements and maintains policies and procedures for the client record keeping of the Agency.  Assists the Home Health Manager and RN case managers with admissions, transfers and discharge of residents. Essential job responsibilities include:

  • Coordinates client care with staff to address client care issues
  • Reports changes or problems in resident condition to the Home Care RN
  • Initiates emergency treatments and reports to Home Care RN
  • Participates in the completion and review of nursing care plans
  • Assists with administration of physician orders
  • Ensures accuracy and promptness in transcribing physician orders
  • Carries out proper charting practices and procedures
  • Consults with RN about client conditions and treatments
  • Orders and administers medications


Minimum Required Qualifications:

  • Degree from an accredited school of nursing for the Licensed Practical Nurse program
  • Current LPN licensure in the state in which they are working
  • Working knowledge of the state requirements for the Comprehensive Home Care license (and other related laws)
  • Up to 2 years’ experience as LPN.
  • Ability to communicate effectively in both written and verbal formats.
  • Basic computer skills.
  • Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations


Preferred Qualifications:

  • Previous experience providing senior care / healthcare setting
